The second step in the application process is the Application for Admission to the School of Optometry.
Approximately three weeks after your on-line submission, you will receive an e-mail from the University of Waterloo with an Application for Admission to the School of Optometry as a PDF attachment (requires Adobe Reader; link at bottom of page). You will fill the form electronically and print a copy. Mail the printed Application for Admission to the School of Optometry and all supporting material directly to the School of Optometry by the deadlines. Verify deadlines on our Important Dates page.
Again, you are encouraged to submit the application and supporting documents early.
Application
The Application contains sections for:
Personal ID
Here you'll supply your name, address, contact information, citizenship, etc.
Academic Record
For each prerequisite course, list the course id, university, year, grade and a web reference to a description in the university's course calendar.
Autobiographic Sketch
The Admissions Committee is interested in learning more about your interests and achievements outside the classroom. The autobiographical sketch provides an opportunity to describe your non-academic profile, including extracurricular activities, non-academic awards, hobbies, work and volunteer experience, athletic involvement, community leadership, club membership, and student government etc.
Confidential Assessment Forms
You’ll need to arrange two confidential references (from individuals who are not your relatives) from:
- An optometrist
- An academic (university professor or instructor)
Completion of coursework
Please note: Typically, you will begin the application process before you have completed all the prerequisite courses. You are responsible for completing the remaining courses and having your transcripts submitted to the School by the deadlines.
Courses completed between the end of May and the beginning of September will not be considered by the Admissions Committee.
Applicants who have not completed all the prerequisites will not be admitted. There are no exceptions. There are no conditional offers of admission.
